Patient Rights and Responsibilities

As a patient of Alliance HealthCare Radiology of West Des Moines, you have certain patient rights and responsibilities including the right to:

  • Receive information at the time of service about your patient rights and how to resolve complaints concerning quality of service.
  • Be treated in a caring and polite way without discrimination as to age, race, color, religion, sex, national origin, disability, sexual orientation or ability to pay.
  • Safeguard the right to privacy, within the law, with respect to confidentiality of disclosures. Case discussion, consultations, examination and treatment are confidential and shall be conducted with privacy maintained.
  • Know all the facts we have about your illness or injury. Your referring physician will give these facts to you after receiving the report from your exam.
  • Know the names of the people taking care of you, as well as how they will help you or provide care for you.
  • Say yes to examinations. You also have the right to say no to or refuse examinations.
  • Be examined in private by your doctor or other health care giver. You also have the right to talk to your doctor in private.
  • Have reports generated to your referring physician in a timely manner.
  • Receive a complimentary CD or films of your images for your next appointment with your referring physician. You may request additional copies for a reasonable fee.
  • Expect that we will do our best to get back to you as quickly as possible when you ask us to do something.
  • Obtain a copy of your health information from the center or examine the record’s health care information with a designated staff member during regular business hours. The center adheres to the free schedule for copying health care information contained in state statutes.
  • Have an opportunity to examine and receive an explanation of your bill, regardless of payment source.
  • Know center rules and regulations as they apply to you as a patient or parent.
  • Tell us when something is seriously wrong. If you present a complaint, your care will not be adversely affected. If you have a problem that you cannot solve with your doctor, nurse or other caregiver, contact Laurie McConville – Manager of Operations. lmcconville@allianceimaging.com or 312-593-3741.
If, after these steps, your problem has still not been resolved, you may register a formal complaint with the Joint Commission’s Office of Quality Monitoring by e-mailing Complaint@JointCommission.org or calling 1-800-994-6610.
You may also file a complaint about a physician or physician’s assistant at: Iowa Board of Medicine, 400 SW Eighth St., Suite C, Des Moines, IA 50309 or at the Board of Medicine’s Web site.

For patients who have not yet reached the age of maturity, the above rights apply to the patients’ parents or guardians. To ensure the best possible care, you have certain patient rights and responbilities, including the responsibility to:

  • Be honest with us and tell us all you know about your present condition, including other studies you’ve had and anything else about your health that would help us.
  • Tell us if you don’t understand anything our staff might say, or if you don’t understand what they might ask you to do. Also, tell us if you think you won’t be able to do what is asked of you during your exam.
  • Respect the right of other patients to receive medical care without being disturbed.
  • Observe the “no smoking” rules of this practice.
  • Give our staff all the information they need regarding payment for your medical care.
